  • We always recommend patients confirm with their insurance regarding services covered at our office. However, below is a list (subject to change) of insurances we participate with as well as the known plans we do not partcipate with.

    If you have a Medicare Advantage Plan, please call your insurance to find out if they will cover our services, provide our Tax ID# 13-3457651

  • We offer competitive and affordable rates for patients whose insurance we do not participate with.

    Call our experienced billing department for a breakdown of costs, Ph# 845-290-8625.

  • If you have HIP Insurance or Tricare Insurance your plan may require a referral from your Provider. Call your insurance company to find out what is needed.

    For all insurances that do not require a referral, we ask you to send the medical records that support your diagnosis and the reason for this visit.

  • Please bring your insurance card, a photo ID, and all pertinent medical records to your first appointment.

    We will provide you with a thin new patient packet that you can complete upon arrival.
